삼진제약(주), 항에이즈 신물질, 해외 유수 에이즈치료제 개발에 활용, 연구 성과 속속 발표 · 발표1. 세계적 에이즈치료제社 美 길리야드(Gilead) 항HIV 물질과 삼진제약 피리미딘디온 화합물을 결합, 새로운 겔(gel)타입 예방제 연구 결과 발표(Journal of Pharmaceutical Sciences) · 발표2. 약물전달시스템 연구 권위자인 유타大 카이저 박사팀, 혁신적인 링 타입 약물전달 시스템에 삼진제약 항에이즈 화합물 탑재 실험 성공 논문 발표 (Antimicrobial Agents and Chemotherapy) 에이즈 예방을 위한 백신 개발이 요원한 상황에서 에이즈 확산 방지 및 치료 효율성을 높이기 위한 다양한 연구가 전 세계적으로 진행되는 가운데, 삼진제약의 항에이즈 신물질‘피리미딘디온’화합물을 활용한 항에이즈 치료제 개발 연구 성과가 해외에서 연이어 발표돼 관심을 모은다. 현재 에이즈 확산 방지를 위해 연구가 활발한 분야중 하나가 바로 여성 외용 예방제 개발을 통한 에이즈 전파 예방 대책이다. 삼진제약의 에이즈치료제 개발 전략적 제휴사인 美 임퀘스트사는 최근 세계적인 에이즈치료제 회사인 미국 길리야드(Gilead)의 에이즈치료제 테노포비어(Tenofovir)와 삼진제약의 항에이즈 신물질인 피리미딘디온(Pyrimidinedione.IQP-0528)화합물을 혼합한 겔(GEL)형태의 새로운 제제가 국소용 여성 예방제로서의 가능성을 열었다고 발표했다.(2012.Journal of Pharmaceutical Sciences). 연구팀은 삼진제약의 피리미딘디온 화합물과 길리야드의 테노포비어 제제가 서로 다른 기전에 의해 상승효과를 낼 수 있다는데 착안, 이 두 물질을 겔 형태로 혼합하여 여성에게 도포했을 때 이물감과 독성이 없고, 매우 안정적이고 탁월한 항HIV 효과를 보인다고 밝혔다. 미 임퀘스트사, 유타大 등 연구기관과 과학자들이 참여한 이번 연구는 삼진과 길리야드의 항에이즈 신물질의 결합이 에이즈 바이러스 차단과 전파 방지에 잠재적 대안임을 확인한 연구 결과로 평가받는다. 삼진제약 피리미딘디온 화합물을 탑재한 새로운 약물 전달시스템 연구도 발표됐다. 약물전달시스템 분야의 세계적인 석학인 미국 유타大 패트릭 카이저(Patrick F. Kiser) 박사팀은 독창적으로 개발한 링(ring)타입의‘고분자 콘돔’약물전달시스템에 삼진제약의 항에이즈 화합물(미국개발명 : IQP-0528, IQP-0532)을 탑재한 연구 결과를 발표했다. 링타입의 고분자 콘돔은 여성의 질 내부 산성도에 맞춰 장기간 약물을 방출하는 형태로, 성관계 시 정자가 들어왔을 때 반고체 상태로 굳으면서 에이즈 바이러스를 잡는 원리다. 카이저 박사팀은 이러한 고분자 콘돔에 삼진제약의 피리미딘디온(Pyrimidinedione) 화합물을 탑재한 결과, 높은 온도에서도 90일 이상 장기간 동안 약물 방출이 안정적이었고, 에이즈 바이러스를 막는 효과도 매우 탁월하다고 밝혔다. 질 내 사용이 편리하고 정상 세균에 해를 끼치지 않으면서 고온에서도 안정적으로 약물 방출이 가능하다는 점은 향후 아프리카 등 열대지방의 에이즈 확산 방지에 적용이 가능하다는 점에서 중요한 의미를 갖는다. 카이저 박사팀은 이러한 연구결과를 Antimicrobial Agents and Chemotherapy 최근호에 발표했다. 이같이 삼진제약의 항에이즈신물질인 피리미딘디온(Pyrimidinedione) 화합물이 세계적 에이즈치료제 회사인 길리야드社의 항에이즈 제제와의 병용 연구 및 혁신적인 링 타입 약물전달 시스템 등 다양한 연구에 속속 채택된 것은 에이즈 바이러스 사멸과 억제 등 효과성, 안전성에서 그 독창적 가치를 폭넓게 인정받고 있다는 증거이며, 이러한 연구 성과들은 향후 진보된 에이즈 치료제 개발에 중요한 밑거름이 될 것으로 전망된다. Development of a combination microbicide gel formulation containing IQP-0528 and tenofovir for the prevention of HIV infection ABSTRACT In light of the increasing worldwide AIDS pandemic, there is a continuing need to develop new prevention strategies to inhibit the transmission of HIV-1. In the absence of a successful vaccine, topical microbicides represent the best strategies to reduce the epidemic. Following the success of HIV therapeutic cocktail strategies, combinations of microbicides including nucleotide reverse transcriptase inhibitors (NtRTIs) and nonnucleoside reverse transcriptase inhibitors (NNRTIs) may offer significant protection from infection over single agents. We have developed a combination microbicide gel formulation for the delivery of IQP-0528, a novel NNRTI, and tenofovir (TFV), a NtRTI. Gel formulations were evaluated based on quantitative viscoelastic and physiochemical evaluations defined by a target product profile (TPP). For the majority of the evaluations, the gel formulations behaved similarly all showed shear thinning behavior, were stable, nontoxic, and active against HIV-1 infection. Gel formulation F2759 displayed increased drug release of 289 ± 100 μg/(cm2 h1/2) and a tissue permeability of 60 times the half maximal effective concentration (EC50) of TFV and 800 times the EC50 of IQP-0528. In addition, F2759 showed osmolality within TPP and the highest performance in gel spreading. We have identified a gel formulation to deliver a combination microbicide of IQP-0528 and TFV that has significant potential to prevent infection of HIV-1. Safe and Sustained Vaginal Delivery of Pyrimidinedione HIV-1 Inhibitors from Polyurethane Intravaginal Rings ABSTRACT The potent antiretroviral pyrimidinediones IQP-0528 (PYD1) or IQP-0532 (PYD2) were formulated in polyurethane intravaginal rings (IVRs) as prophylactic drug delivery systems to prevent the sexual transmission of HIV-1. To aid in the selection of a pyrimidinedione candidate and the optimal loading of the drug in the IVR delivery system, four pyrimidinedione IVR formulations (PYD10.5wt%, PYD11wt%, PYD24wt%, and PYD214wt%) were evaluated in pigtailed macaques over 28 days for safety and pyrimidinedione vaginal biodistribution. Kinetic analysis of vaginal proinflammatory cytokines, native microflora, and drug levels suggested that all formulations were safe, but only the high-loaded PYD214wt% IVR demonstrated consistently high pyrimidinedione vaginal fluid and tissue levels over the 28-day study. This formulation delivered drug in excess of 10 μg/mL to vaginal fluid and 1 μg/g to vaginal tissue - over 1000 times the in vitro EC50. The in vitro release of PYD1 and PYD2 under non-sink conditions correlated well with in vivo release, both in amount and kinetic profile, and therefore may serve as a more biologically relevant means of evaluating release in vitro than typically employed sink conditions. Lastly, the pyrimidinediones in the IVR formulation were chemically stable after 90 days of storage at elevated temperature, and the potent nanomolar antiviral activity of both molecules was retained after in vitro release. Altogether, these results point to the successful IVR formulation and vaginal biodistribution of the pyrimidinediones and demonstrate the usefulness of the pigtail macaque model in evaluating and screening antiretroviral IVR formulations prior to preclinical and clinical evaluation.
