ILIAS Biologics , a company that develops exosome-based precision treatment, announced on the 29th that it succeeded in phase 1 clinical trial of ILB-202, an anti-inflammatory exosome new drug candidate under development, and published the results in the September 2025 issue of the Journal of Extracular Vesicles (JEV), an official journal of the International Society for Extracular Vesicles (ISEV). This clinical trial was conducted with randomized, double-blinded, placebo-controlled phase 1 clinical trials in healthy people, and ILB-202 proved safety and tolerability in all dose groups, especially as a result of single-cell RNA sequencing analysis of blood obtained before and after drug administration, ILB-202 selectively inhibits nuclear factor kappa B (hereinafter NF-κB), a key pathway for inflammatory responses, mainly involved in inflammatory responses, and at the same time, activates anti-inflammatory signaling pathways such as TGF-β and visfatin to induce immune homeostasis recovery. In addition, unlike the existing NF-κB inhibitors that were concerned about side effects by suppressing the overall immune function, ILB-202 was also confirmed to have differentiated action properties that target only pathologically excessively increased inflammation while maintaining physiological immunity. ILIAS is currently in the process of developing and consulting protocols for the entry into follow-up clinical trials of ILB-202, and is speeding up the industrialization and commercialization of next-generation exosome drugs through the expansion of multiple immune and inflammatory disease pipelines based on unique exosome platforms.
