９価HPVワクチン「シルガード®９水性懸濁筋注シリンジ」男性への接種対象拡大と肛門がんの予防の適応追加について承認を申請

2024/11/15 15:00 JST 報道関係各位 MSD株式会社 MSD株式会社（本社：東京都千代田区、代表取締役社長：カイル・タトル、以下 「MSD」）は、本日、ヒトパピローマウイルス（HPV）の9つの型に対応した「シルガード®9水性懸濁筋注シリンジ（一般名：組換え沈降9価ヒトパピローマウイルス様粒子ワクチン（酵母由来）、以下「シルガード®9」）」について、以下に関する製造販売承認事項一部変更承認申請を行いましたのでお知らせいたします。 9歳以上の男性への接種対象拡大と、9歳以上15歳未満の男性に対する2回接種の「用法及び用量」の追加 肛門癌及びその前駆病変と、男性での尖圭コンジローマの予防の適応追加 現在、日本で男性に接種可能なHPVワクチンは、４価HPVワクチン「ガーダシル®水性懸濁筋注シリンジ」のみですが、今回の申請が承認された場合には、さらに5つのHPV型を加えた9つの型に対応している9価HPVワクチン「シルガード®9」でも、肛門がんと尖圭コンジローマの予防のための男性への接種が可能となります。2024年8月時点で「シルガード®9」は、90以上の国または地域で女性だけでなく男性への適応も取得しています ※1 。また、HPVワクチンは、2024年10月時点で70以上の国または地域で定期接種プログラム等を通して男女ともに接種が推奨されています ※2 。若年層に対する2回接種は、米国やドイツ、フランスなどでも承認されており ※3,4 、今回の申請で日本も諸外国と同様の性別によらないHPVワクチン接種の環境が、より一層整うことが期待されます。 さらに、今回の申請が承認された場合、女性においては、子宮頸がんや尖圭コンジローマなどの従来の適応症に、新たに肛門がんが追加されることになります。肛門がんは、その約80～90％にHPV感染が関与しているとの報告があります ※5,6 。また、肛門がんは、現時点では検診などの早期発見のためのスクリーニング方法が確立されておらず、HPVワクチン接種は、肛門がんおよびその前駆病変の予防のための有用な手段であると考えられています。 MSDは、今後も日本の人々をHPV関連がんおよび疾患から守り、公衆衛生の向上に貢献できるよう全力を尽くしてまいります。 「シルガード®9」について 「シルガード®9」は、9つのHPV型（6、11、16、18、31、33、45、52、58型）に対応した9価HPVワクチンです。これら9つのHPV型のうち、HPV16、18、31、33、45、52、58型の7つの型は、子宮頸がん、肛門がんなどの原因となることが知られています ※7 。子宮頸がんで検出されたHPV型のうち、これら7つの型が88.2%を占めていたという報告があります ※8 。また、肛門や性器周辺に良性のイボが発生する尖圭コンジローマでは、その86%でHPV6、11型が検出されたという報告があります ※9 。9価HPVワクチンは、2014年12月に世界で初めて米国で承認され、日本では9歳以上の女性を対象に子宮頸がんなどの予防を「効能又は効果」として、合計3回接種する「用法及び用量」で、2020年7月21日に製造販売承認を取得しました。また、9歳以上15歳未満の女性に対する2回接種の「用法及び用量」の承認は、2023年3月8日に取得しました。 ※1 社内資料 ※2 https://app.powerbi.com/view?r=eyJrIjoiNDIxZTFkZGUtMDQ1Ny00MDZkLThiZDktYWFlYTdkOGU2NDcwIiwidCI6ImY2MTBjMGI3LWJkMjQtNGIzOS04MTBiLTNkYzI4MGFmYjU5MCIsImMiOjh9 (Accessed on Oct 23, 2024) ※3 https://www.fda.gov/media/90064/download?attachment ※4 https://www.ema.europa.eu/en/documents/product-information/gardasil-9-epar-product-information_en.pdf ※5 Ouhoummane N et al.
